Presenter's mod_form is throwings numerous strictness warnings

      mod_form.php is throwing heaps of PHP E_STRICT type warnings.

      Workaround: in mod_form.php, replace all instances of
      &MoodleQuickForm::createElement
      with
      $mform->createElement
