In the rate questions reports I am proposing to add to the the current "Average" display a second display, showing the number of responses per proposed choice.
I am attaching a patch against latest moodle 2.1 version and a screenshot.

This display improvement does not involve any changes to the database, is relatively simple to implement, and has been thoroughly tested (but more tests are welcome).
